Hospital-onset bacteremia and fungemia (HOB) rate has emerged as an important quality metric, offering hospitals and healthcare system administrators a standardized way to monitor serious hospital-acquired bloodstream infections that are not currently being reported. HOB serves as an indicator of broader systemic issues, including gaps in diagnostic stewardship, infection prevention practices, and environmental safety. HOB tracking allows hospital leaders to identify vulnerabilities, implement targeted interventions, and ultimately enhance patient safety and care quality.

A cornerstone of effective HOB management is robust diagnostic stewardship—ensuring the appropriate ordering, collection, and interpretation of diagnostic tests, particularly blood cultures. Diagnostic stewardship aims to reduce blood culture contamination by promoting evidence-based collection practices and ensuring testing is conducted for the right patients at the right time and in the right way. 

LEARNING OBJECTIVES:

1- Identify the driving factors for the HOB measure development and planned implementation.

2. Recognize the importance of diagnostic stewardship related to the root causes and preventability of HOB.

3. Explain the impact of poor diagnostic stewardship and blood culture contamination as one of the major causes of (false-positive) HOB.

4. Differentiate between skin common commensal and pathogenic organisms with blood culture contamination.

5. Estimate the clinical and economic impact that diagnostic error with blood culture contamination may have on the patient, the hospital and public health
